Teams skip discovery

Under time pressure, teams jump straight to solutions before genuinely understanding the problem. The result: elegant answers to the wrong questions.

Insights disappear between phases

Research done in Discover rarely survives into Develop. Teams start ideating without the evidence they gathered, repeating old assumptions.
